Position Summary

Mitchell County Public Health & Home Health is seeking a compassionate, self-motivated Registered Nurse (RN) to serve in a dual role providing both skilled home health nursing and public health nursing services.  This position supports individuals in their homes with skilled nursing care while also assisting with county-wide public health programs focused on prevention, outreach, and community wellness.

Key Responsibilities

Public Health Nursing Duties

  • Assist with public health programs including communicable disease follow-up, chronic disease prevention, immunizations, maternal/child health, and community outreach.
  • Conduct case interviews, contact tracing, and client education during communicable disease investigations.
  • Provide community-based health education on chronic disease, immunizations, environmental health risks, substance use, and prevention strategies.
  • Support public health emergency response efforts during outbreaks, disasters, and community events.
  • Offer basic nursing services such as blood pressure screenings, immunizations, and health assessments within scope of public health practice.
  • Facilitate referrals to medical, dental, behavioral health, and social support resources.
  • Participate in community events, school programs, health fairs, and health promotion initiatives.
  • Develop and distribute culturally appropriate health education materials.
  • Maintain accurate and confidential records, documentation, and data as required.
  • Assist with data collection, program evaluation, and quality improvement efforts.

 

Home Health Nursing Duties

  • Provide skilled nursing care in accordance with physician orders and individualized plan of care.
  • Complete initial assessment visits and ongoing reassessments of patient needs.
  • Initiate, implement, and revise plans of care as needed.
  • Deliver nursing services requiring specialized clinical judgment and skills, including preventive and rehabilitative procedures.
  • Prepare accurate and timely clinical notes, visit documentation, and care summaries.
  • Monitor and communicate patient condition changes to providers and agency leadership.
  • Counsel patients and families regarding care needs, medications, and disease management.
  • Coordinate interdisciplinary care and refer to PT, OT, SLP, or other services as appropriate.
  • Supervise, teach, and support Home Health Aides.
  • Participate in care conferences, in-service trainings, peer review, and quality improvement activities.
  • Complete OASIS assessments and related documentation within required timeframes.
  • Participate in scheduled on-call rotation as assigned.
     

Qualifications

  • Current Iowa Registered Nurse (RN) license in good standing.
  • Graduate of an accredited school of professional nursing.
  • Two years of nursing experience preferred.
  • Strong clinical assessment skills and ability to exercise sound judgment.
  • Commitment to public health principles, health promotion, and equitable access to care.
  • Ability to work independently, manage time effectively, and adapt to changing priorities.
  • Strong communication skills and ability to build trust with diverse populations.
  • Acceptance of mission, vision, & values of Mitchell County Public Health & Home Health.
     

Work Environment

  • Provides care in client homes, the agency office, and various community settings.
  • Requires travel within the county; mileage reimbursement provided. Some out of county travel for scheduled trainings & conferences.
  • Involves moderate to heavy lifting, extended standing, bending, and physical activity as needed in both home health and public health roles.
     

Supervision

  • Home Health Duties: Supervised by Clinical Manager/Administrator.
  • Public Health Duties: Supervised by Public Health Coordinator/Administrator.
